Nehalem Bay Health Center & Pharmacy offers a Sliding Fee Discount Program to help make healthcare more affordable.
Eligible patients may receive a discount based on household income and family size. Patients may apply whether they have insurance or not.
No one will be denied services because of an inability to pay.
All patients may apply for the Sliding Fee Discount Program, including patients who:
Eligibility is based on household income and family size, not insurance status.
Your discount level is determined using the current Federal Poverty Guidelines and NBHCP’s approved Sliding Fee Discount Schedule.
The chart should include:
Patients with household income above 200% of the Federal Poverty Guidelines do not qualify for a sliding fee discount.
Complete the Sliding Fee Discount Program application and provide the household and income information requested on the form.

We will contact you if additional information or documentation is needed. Once your application has been reviewed, you will be notified of your discount level.

If you have insurance, we encourage you to use it — the Sliding Fee Discount Program is designed to support patients who are uninsured or underinsured. Yes. Patients with insurance may still qualify for reduced out-of-pocket costs based on household income and family size. Discounts may be subject to insurance requirements.
