Licensed Clinical Social Worker

Position Overview

Nehalem Bay Health Center & Pharmacy, located on the beautiful North Oregon Coast, has an opening for a full-time or part-time Licensed Clinical Social Worker.

This fully benefited position includes medical, dental, and vision coverage beginning on the first day of employment. This position provides direct patient care, primarily to students and staff at the Neah-Kah-Nie School District Student Health & Wellness Center. The role offers mental health support for patients as part of their overall care, helps achieve patient clinical measures, and contributes to the wellness of the community.

Duties

• Collaborate with the primary care provider in recognizing, treating, and managing mental health and psychosocial issues of patients.
• Act as a contributing member of the primary care team.
• Conduct patient intakes, including diagnostic and functional evaluations.
• Make recommendations to the primary care provider regarding treatment goals and behavioral health needs.
• Provide consultation and training to primary care providers to support treatment of mental health concerns.
• Provide long-term and brief interventions for patients in need of mental health services.
• Provide timely feedback to primary care providers about patient care, treatment recommendations, and progress.
• Advise providers about which patients are better served in primary care and which should be referred to specialty mental health services or other resources.
• Initiate follow-up to determine how patients are doing and whether changes in treatment approaches are needed.
• Develop relapse prevention plans when indicated.
• Help patients maintain stable functioning.
• Assist in identifying at-risk patients and developing plans to prevent worsening conditions.
• Monitor and coordinate behavioral health-related services, including connections with outside treatment providers when appropriate and with patient permission.
• Assist patients with community functioning, including public benefits, vocational rehabilitation, social support, housing, and related needs when feasible.
• Help patients make healthier lifestyle choices when behavioral changes may improve physical health.
• Provide self-management skills and education so patients can participate in their own treatment and recovery.
• Help patients cope with chronic conditions such as pain and diabetes.
• Provide consultation to clinic management and team members about behavioral services, outcomes, and program evaluation.
• Work with other team members to provide a comprehensive approach to patient wellness.
• Support efforts to improve equitable access to healthcare.
• Perform other related duties as assigned.

Skills and Attributes

• Knowledge of the principles and practice of social work.
• Knowledge of state and federal laws pertaining to social workers and HHS/HRSA regulations.
• Proficiency in identifying and treating mental disorders.
• Ability to think in terms of population management.
• Knowledge of evidence-based behavioral assessments and interventions relevant to medical conditions.
• Ability to make quick and accurate clinical assessments.
• Skilled in care management and familiar with local referral resources.
• Skilled in targeted, brief psychotherapy and group sessions.
• Knowledge of basic physiology, psychopharmacology, and medical terminology.
• Ability to document services in a way that is useful to primary care providers and management for quality improvement.
• Active listener interested in being part of a collaborative and patient-centered team.
• Ability to show empathy and understanding toward all patients, including those with special needs.
• Ability to recognize, respect, and work effectively with people from diverse social, cultural, and ethnic backgrounds.
• Skill in establishing and maintaining effective working relationships with staff, board members, patients, and community members.

Qualifications

• Master of Social Work from an appropriately accredited college or university.
• Current license to practice social work in the State of Oregon.
• Experience working in a behavioral health integration model.
• Experience working with electronic medical record software.

Job Type

Full-time or part-time available.

Expected hours for part-time: 20–30 hours per week.
