This position provides direct patient care, primarily to students and staff at the Neah-Kah-Nie School District Student Health and Wellness Center. This position offers mental health support for patients as part of their overall care, achieving patient clinical measures and contributing to the wellness of the community.
Duties:
Collaborates with the primary care provider in recognizing, treating, and managing mental health and psychosocial issues of patients and acts as a contributing member to the primary care team.
Conducts patient intakes, focusing on diagnostic and functional evaluations, and makes recommendations to the primary care provider concerning the patient’s treatment goals and plan for behavior health needs.
Provides consultation and training to the primary care providers to enhance their skill and effectiveness in treating mental health problems.
Provides long-term as well as brief intervention for patients who are in need of mental health services (be able to offer brief, targeted interventions and be comfortable with short-term counseling).
Gives primary care providers timely feedback about the patient’s care for behavior health needs, treatment recommendations and progress via documentation in the patient’s record and verbal feedback.
Advises the primary care provider about which patients are better served at the primary care setting and which should be referred to specialty mental health facilities or elsewhere.
Initiates follow-up to ascertain how patients are doing and to determine if any changes in treatment approaches are indicated.
Develops, where indicated, relapse prevention plans and helps patients maintain stable functioning.
Assists in the detection of at-risk patients and in the development of plans to prevent worsening of their conditions.
Monitors and coordinates the delivery of health services for patients as related to behavioral health care, including linking with other treatment providers not only within the primary care setting but, with the patient’s permission, outside it as well.
Assists, to the extent feasible, in the patient’s community functioning by helping with public benefits, vocational rehabilitation, social support, housing, etc.
Works, where indicated, to effect behavioral changes in patients with, or at risk for, physical disorders and helps them make healthier lifestyle choices.
Provides patients with self-management skills and educational information needed so they can be full participants in their own treatment and recovery.
Helps patients, where indicated, to cope with chronic conditions like pain and diabetes.
Provides consultation to clinic management and other team members about behavioral services and suggested areas of outcome and program evaluation.
Works with other team members (registered nurse and clinical pharmacist) to provide a comprehensive approach to wellness for the patient.
Supports efforts to improve equity within the community to access to healthcare.

The Patient Services Representative serves patients by greeting and assisting them, scheduling appointments, and maintaining records and accounts within the strictest limits of confidentiality.
Duties:

This is a full-time (40 hours per week) position with Medical, Dental, and Vision benefits effective on the first day of employment. The Medical Assistant will perform duties to assist the patient and clinic providers in the delivery of primary care services as needed and within the parameters of their certification.
Duties:
Prepares patients for healthcare appointments, including greeting and rooming patients, active listening to patients during the interviewing process, recording chief complaint, obtaining vital signs and other lab work as indicated, and preparing patients for individual treatment and appropriate examinations.
Assists providers in healthcare activities, performance of diagnostic and/or therapeutic procedures.
Helps keep providers on schedule; monitors and adjusts patient flow and limits interruptions. Will keep patient updated about their visit, especially when delays may occur in being seen.
Ensures an appropriate environment for delivery of healthcare by maintaining a clean, safe, and orderly workspace; assists in maintenance of all equipment used by the medical assistants and ensures that exam rooms are stocked accordingly.

Dental Assistant I
The Dental Assistant I position provides direct chairside support to the clinic’s Dentist and Dental Hygienist, completing prep work so they can more efficiently care for patients. The Dental Assistant’s duties include completing intake paperwork and updating patient records, sterilizing and organizing dental tools, and assisting during dental procedures.
This position will be part of a new dental care team that will provide dental services at the new and expanded Nehalem Bay Health Center and Pharmacy facility in Wheeler, Oregon. The new facility is expected to open in September 2025.
Duties:
Provides patients and their families with instruction on oral hygiene, oral disease prevention and post-operative instructions, including anticipated next steps and appointment information.
Sets up operatories and portable unit equipment and ensures that instruments, supplies, and equipment are immediately available for patient care.
Obtains and enters patient information into electronic dental record, including chief complaint, health history, medications, allergies, vitals, patient pain level.
Provides direct clinical assistance to Dentists and Dental Hygienists by passing instruments for operative and oral surgery procedures and mixing filling and medicinal materials.
Exposes x-rays and ensures all x-rays are suitable for diagnosis.
Takes oral impressions and pours models or scanned impressions.
Maintains proper infection control practices at all times, including the following areas: PPE, hand hygiene, handling and disposal of sharps and biohazard waste, instrument sterilization, and dental operatory disinfection.
Communicates with Dentist of their daily schedule and patient information; communicate with dental team about schedule changes and informs providers of changes.
Communicates expected wait times to patients, Dentists, and receptionists.
Seats and positions patient in the operatory.
